Aboriginal Health Worker (Identified Role) We are looking for an Aboriginal Health Worker to join our dedicated Primary Health Services team, providing crucial support in the coordination and delivery of the Indigenous Chronic Disease Clinic (ICD) and other health programs across designated regions. This role is based in Wiradjuri Country, Bathurst NSW and is Part-Time. Key Responsibilities: Lead daily operations of the ICD Clinic and provide administrative support. Assist clinicians with client intake, assessments , and follow-ups. Facilitate health education sessions and yarning circles for the community. Identify and address barriers to accessing health services for First Nations people. Support telehealth sessions and stay in contact with clients and health teams. Help improve service delivery through quality improvement activities. Actively contribute to the multi-disciplinary health care team. What You Bring to the Role: Cultural Identity: This is an identified position – you must be Aboriginal or Torres Strait Islander for this role. Knowledge of chronic health challenges in Aboriginal and Torres Strait Islander communities. Experience working with First Nations communities and a connection to the local community. Strong communication skills and ability to adapt services to cultural needs. Proficient computer skills and willingness to learn new systems. Understanding of chronic disease management and referral pathways.
