Job Description The opportunity: We are looking for a Service Connection Team Leader to join our supportive and professional multidisciplinary teams in a Full Time capacity. Responsibilities include but are not limited to; Undertake Assertive Outreach activities relating to connections and linkages with community services, networks and community groups to assist the Connectors to identify and engage with the target populations across the LGAs. Present an energised profile with decisive decision making related to day-to-day management of program and provide clear communication to build a strong, positive focus with your team. Be the day-to-day liaison contact for our partner agency and funding body. Represent healthAbility in the aged care sector, as well as through other relevant networks and working groups. Contribute to writing funding submission applications as appropriate. Ensure program has strong links with external stakeholders, referrers, and clients. Manage the service budget and ensure all funder targets are met. Desired Skills and Experience Demonstrated knowledge of trauma informed practice and cultural safety and evidence-based approaches to improving wellbeing. Experience with building and maintaining effective working relationships internally and with external organisations, networks and community groups. A relevant tertiary qualification - Health, Social Work, Social Sciences. >5 years’ experience working in community/public health or community-based setting. Demonstrated leadership experience. Relevant degree in health, social work or other relevant qualification. Minimum 1 year experience in the CareFinder program is preferred.
