Aboriginal Health Unit Clinical Care Coordinator Are you seeking a diverse and rewarding Clinical Care Coordinator role in a leading regional hospital? Join the Aboriginal Health Unit (AHU) at Latrobe Regional Health, where a ‘world of opportunities awaits'. Latrobe Regional Health (LRH) is looking for a dedicated and experienced Clinical Care Coordinator to join our friendly AHU team on an ongoing, full-time basis . A part-time role would be considered for the successful candidate. The Aboriginal Health Unit Clinical Care Coordinator will coordinate integrated care for patients of Aboriginal and/or Torres Strait Islander descent within our services and facilitate care within the community through collaborative relationships with external stakeholders. The Clinical Care Coordinator will identify, support and provide pathways to access home and community-based services for Aboriginal/Torres Strait Islander patients and will work to reduce the number of Emergency Department re-presentations and unplanned readmissions. The Clinical Care Coordinator will work closely with other health workers to develop and maintain systems supporting the seamless transition of Aboriginal/Torres Strait Islander people with complex medical conditions back into the community. The Clinical Care Coordinator will also work together with other care providers to increase patient and carer's knowledge, confidence, and participation in their self-management, treatment and care planning. About you Ideal candidates for this role will have: Current Registration as an Enrolled Nurse or Registered Nurse (Division 1) with AHPRA. Sound knowledge and understanding of Aboriginal/Torres Strait Islander culture, society and kinship networks, as well as the ability to communicate with, and be accepted by, the Aboriginal/Torres Strait Islander community. Ability to coordinate care for Aboriginal and Torres Strait Islander patients, both in the hospital system, Mental Health setting and in the community. Well-developed interpersonal and communication skills, written and verbal, and the ability to develop and maintain communications with Aboriginal/Torres Strait Islander communities, staff from various Mental Health sectors, and relevant government agencies. Demonstrated ability to interpret and analyse data, and develop and maintain statistical databases. Please note: the desirable candidate would be of Aboriginal/Torres Strait Islander descent, however, this is not essential.
