Aged Care Assessment Team Registered Nurse This position qualifies for a Recruitment Incentive Package up to the value of $10,000 (pro-rata part-time/temporary positions less than 18 months), and a reoccurring Annual Retention Bonus valued up to $10,000 (pro-rata part time), under the NSW Health Rural Health Workforce Incentive Scheme. This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas. Employment Type: Permanent Part TimePosition Classification: Registered NurseRemuneration: $36.38 - $51.08 ph + SuperHours Per Week: 28Requisition ID: REQ542749 The Aged Care Assessor / Specialist Registered Nurse will provide comprehensive assessment services and care planning to determine the overall needs of frail, older people in community, residential and hospital settings within the FWLHD. Minimum Requirements Current registration to practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA). Demonstrated post graduate experience working with older people in an acute, residential or community setting. Demonstrated ability to work independently and in an effective manner encompassing appropriate initiative, organisational, problem solving and time management skills. Proven written and verbal communication skills including demonstrated computer literacy. Demonstrated ability to work autonomously as well as function as a member of a multidisciplinary team. Understanding or experience in the health and functional support needs of older people and their carers in the community setting. Current Drivers Licence (minimum class C) valid within Australia and demonstrated competent driving ability/ experience. Current Working with Children Check, Aged Care Check and NDIS check.
