The Central Coast Local Health District (CCLHD) is currently seeking a dedicated and compassionate Registered Nurse to join our dynamic healthcare team within our Nunyara Aboriginal Health Unit. About You As this is a targeted position that works directly with Aboriginal people, ideally, you are an Aboriginal person or a non-Aboriginal person with a sound understanding of providing culturally sensitive and trauma-informed care for Aboriginal people. You have current Authority to practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA). You have well developed communication skills and enjoy working in a multi-disciplinary team environment. Where you'll be working The position of the Registered Nurse exists within Nunyara Aboriginal Health Unit, as a part of Central Coast Local Health District. This role is responsible for the continuous monitoring of the health status of Aboriginal patients and communities as well as responding to episodic health needs of patients referred to the Aboriginal Chronic Care service. The Registered Nurse will work closely with the Aboriginal Health Unit and develop internal and external partnerships to ensure culturally appropriate and comprehensive service delivery. Primarily the position is to provide case management and support for people with multiple chronic conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or an avoidable hospital admission.
