CLIENT LIAISON / ROSTERING OFFICER - DISABILITY & AGED CARE Caring Approach is a dynamic, vibrant, supportive and caring organisation. We provide tailored in-home support and community-based care for the elderly, people with both physical and intellectual disabilities, and those recently discharged from hospital. We are seeking a caring and professional individual to become part of the Caring Approach team to coordinate services for people living at home. Caring Approach currently has an opportunity for a dynamic individual to join the team as Client Liaison / Rostering Officer in our Head office based in Hornsby. The initial contract is six months with the potential for an extension after this time. We have a great office culture and due to expansion, are looking for a dynamic individual with previous experience, strong administration skills, and the ability to engage with a variety of personality types to join a fun office environment in a rewarding role. Responsibilities: Rostering, coordinating, and assisting in the service management of NDIS Participants and aged care Consumers who have engaged Caring Approach, including the Commonwealth Home Support Program (CHSP). Meeting and engaging with new clients to explain what NDIS and Aged Care services Caring Approach can provide. Engaging subcontractors where required. Developing new service agreements and gathering required intake information and documents to ensure a safe service is provided and meets the wants and needs of the person who has engaged us to support them. Liaising with clients & staff to coordinate services in line with the needs of the person, funding, and Caring Approach guidelines. Developing care plans and service duties. Solving roster issues and reporting incidents through Caring Approach's after-hours service and incident reporting process. Availability to be on call after hours when required. To be successful in this role you will need: An understanding of coordinating and delivering care in a person-centred way. Experience in rostering and scheduling of services. Flexibility in your approach and willingness to assist fellow team members in other program areas such as aged care where required. A thorough understanding of NDIS and the Aged Care system including CHSP. A thorough understanding of the SCHADS award and rostering principles. Experience in rostering/scheduling services for multiple clients in a community setting. Strong administrative and time management skills including facilitating team meetings, creating agendas, and taking minutes. Familiarity with Microsoft applications and the DEPUTY rostering platform or a willingness to learn. The ability to problem-solve and find solutions when issues arise. Ability to engage positively with a variety of personality types including those who suffer from mental health conditions and complex health conditions. Ability to identify staff training needs for specific services. Ability to think outside the box and have excellent time management skills. Ability to read, interpret, identify, and develop service information. An understanding of the NDIS Quality & Safeguards Commission and the Aged Care Quality & Safety Commission framework. Ability to manage and escalate complaints and feedback. Appropriate visa to work in Australia. Current Police Check within the last 12 months or willingness to obtain prior to employment. Working with Children Check (WWCC). NDIS Worker Check (NDISWC). 12 months minimum experience in a similar role. Current First Aid/CPR certificate. Driver's licence as travel to meetings may be required. Empathetic and understanding approach to providing care to people with complex needs including mental health. Completion of the NDIS new worker module or willingness to complete as part of the organisation's induction process. Completion of the Covid-19 e-learning documents. An understanding of providing advice to front-line team members with our after-hours service. Up to date with Caring Approach vaccination requirements including COVID-19. Experience in working with people to increase their independence in a community-based setting. Experience in the aged care sector including CHSP. Background in nursing or other relevant discipline. Ability to speak a second language other than English. Ability to provide guidance to front-line team members to implement care plans and support strategies as part of the services.
