If you’re ready to join a vibrant team and make some real waves in the community, read on!Position Overview We’re looking for a passionate and energetic Care Coordinator to join our team in Griffith, NSW (Wiradjuri Country) to help bridge the gap between individuals in need and the local services that can empower them. You’ll be the superhero, by reviewing health needs connecting them to required health services, building health literacy, and developing capacity in the self-management of health conditions.Key Responsibilities Build Connections: Forge strong relationships with local service providers to make access smoother than ever. Personalised Care: Get to know your clients, assess their needs, and whip up tailored health care plans that pack a punch. Be an Advocate: Support clients within the healthcare system and connect them to vital primary care services. Collaborate & Innovate: Team up with community organisations to identify opportunities for social connection and participation. Track Progress: Keep an eye on client journeys, adjusting care plans as they evolve and thrive. Provide Support: Offer professional guidance and encouragement every step of the way. Qualifications Registration as an Enrolled or Registered nurse with the Nursing and Midwifery Board of Australia (NMBA) and/or qualified Aboriginal Health Worker. Stellar communication skills that shine and the ability to engage and communicate effectively with Aboriginal and Torres Strait Islander people and communities and people from culturally and linguistically diverse (CALD) backgrounds. Demonstrated ability to comprehensively conduct assessments, develop a plan for care coordination; provide safe, appropriate and responsive quality practice and evaluate outcomes. Demonstrated competencies in and/or knowledge of the complexities and challenges involved in caring for individuals with complexities, including, but not limited to: chronic care conditions, mental health, trauma, and alcohol and other drugs concerns.
