Your Role As a Care Coordinator , you'll provide advanced clinical leadership and support to ensure exceptional care for residents with complex needs. You'll oversee assessments, care planning, and service coordination, ensuring compliance with organisational and regulatory standards. We have an exciting opportunity for a Care Coordinator to join our team on a full-time basis, 5 days per week, Monday to Friday 7am – 3pm, working 76 hours per fortnight. Located 6km West of Caloundra's CBD, our 96-bed service is nestled within the beautiful town of Little Mountain, offering a combination of coastal and country living. Key Responsibilities: Oversee comprehensive assessments of residents’ needs and well-being, ensuring plans reflect their goals and preferences; Develop, implement and review individualised care plans to meet evolving requirements; Coordinate external service providers to deliver seamless care for residents; Lead the collection and submission of accurate data for compliance and reporting purposes; Lead a team of care professionals to deliver high-quality, person-centred care; Ensure accurate documentation and compliance with aged care standards; Actively participate in the review and improvement of clinical programs and initiatives; Shape a culture of excellence by mentoring and guiding your team. What We’re Looking For Registered Nurse (AHPRA registration) with three years of aged care or clinical experience. Strong leadership skills to inspire and manage diverse teams effectively. Excellent communication, problem-solving, and organisational abilities. A commitment to safety, quality, and continuous improvement. Possess a current National Police Check, NDIS Worker Screening Check, First Aid and CPR certifications (or ability to obtain). Why Join Us?
