The Opportunity We are looking for experienced Registered and Enrolled Nurses to join our Clinical Programs team as Care Coordinators to support the After Hospital Home Care (AHHC) program. AHHC is a short-term service for eligible Medibank members with chronic health conditions and certain accompanying risk factors to transition to and recover at home following a hospital admission. Working closely with the team and reporting to the Program Lead, you will ensure a comprehensive patient-centred management plan is achieved by: Conducting initial phone assessment and providing information about the program. Arranging a time for their first in-home visit, attended by a registered nurse, to identify risks that may impact the patient’s recovery at home. Scheduling future visits with the registered nurse and/or allied health professional/s, as required. Monitoring patient progress and managing discharge planning including referral to other services as appropriate. Building and maintaining effective relationships with a variety of different stakeholders and healthcare professionals. Provision of telephone support, education, and advice. This is a permanent part-time (0.6) up to full-time position working across Mon-Fri with shifts available 7.30 am - 6.30 pm and can be work from home or at one of our offices or a combination of both. About You As an outstanding communicator, your empathetic and intuitive people skills will be central to your success. You are naturally proactive and goal-focused, and at all times, you take a patient-centred approach to achieve positive outcomes. You will also have: Minimum 2 years' experience as a Registered or Enrolled Nurse with unrestricted AHPRA registration. Strong customer service orientation and relationship management. Excellent clinical reasoning skills. Ability to work in a fast-paced environment. Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
