We also offer a range of residential and nursing care settings for older people with varying levels of need.Cleveland House are currently recruiting for a Care Home Assistant Practitioner to join their dedicated team in Southport. As a Care Home Assistant Practitioner, you will be supervising Support Workers who assist our service users in achieving their outcomes in the way that they choose.You will work on a variety of duties, including: Deliver prescribed care to a defined group of patients/Service users that reflect their individual needs. Assess, plan and deliver, with the patient and their carer/family, care which takes account of patient choice and wishes. Contribute to a team approach to patient care in conjunction with all members of the Multi-Disciplinary team and liaise effectively with external professionals. Maintain accurate and contemporaneous records of patient/service user care delivered. Respect and apply the requirements of equality and diversity, promoting and role modelling these across the team. Develop and maintain a good professional relationship with patients, colleagues, and visitors responding promptly and courteously to requests, enquiries, concerns, complaints, and suggestions.
