HealthWISE is seeking a Care Manager to play a vital role in shaping a new Multidisciplinary Team (MDT) model for diabetes care in the Moree region. Why Join Us? Lead an innovative healthcare model improving diabetes care in rural NSW Work alongside experienced allied health professionals, GPs, and service providers Be part of a team dedicated to creating healthier communities Competitive salary under the HealthWISE Enterprise Agreement Your Role: As the Care Manager, you will be the linchpin of our MDT, ensuring seamless coordination of care between general practitioners, allied health providers, and community health organisations. You will: Establish and facilitate fortnightly MDT clinics Act as a single point of contact for the team and patients Ensure clients are equipped to follow their care plans Lead group education sessions to promote healthy lifestyles Monitor and evaluate the impact of the project Build strong relationships with local health providers and Aboriginal health services What We’re Looking For: Registered health professional (AHPRA or other recognised peak body) Strong ability to collaborate and coordinate across teams and organisations Passion for client-focused, person-centred care Experience in facilitating group education and supporting behaviour change Cultural awareness and commitment to Aboriginal health initiatives Ability to work both independently and as part of a team Willingness to travel and work after hours if required Experience in diabetes care and program management Remote work experience with high accountability Requirements: Right to work in Australia National Police Check & Working with Children Check/Blue Card Full Position Description: View Position Description For more information, please contact Tanya Hague at 0447 811 466 or via email at Role Type Pay Rate 35 AUD - 45 AUD (Hour) Company Overview HealthWISE provides primary health care programs and clinical services across the Darling Downs West Moreton and Goondiwindi regions of QLD and the New England North West area of NSW.
