Care Manager (Home Care Packages)

We are BCR Communities, a not-for-profit award winning community organisation serving communities within the Illawarra and Shoalhaven for over 30 years (formerly Bay & Basin Community Resources). We are looking for an experienced Care Manager to join our team and deliver person-centered support to clients receiving in Home Care Packages (HCP). This role is full time, Monday through Friday, and can be based flexibly between Nowra and Sanctuary Point with the option of a hybrid work from home arrangement. The role also involves visits to clients in their homes and in the community. You will be supporting clients to develop healthy ageing and wellness goals in their care plans, in accordance with the Aged Care Quality Standards. You will achieve this through working in a multidisciplinary team, forming strong professional relationships with clients and their support network, whilst working closely with all suppliers and health professionals involved in their care. We are specifically seeking someone who will bring a track record of supporting ageing clients with diverse and complex care needs, including mental health and dementia-related behavioural aspects. To be successful in this role you will be someone who: Collaborates with clients, listening with empathy to their needs, goals and preferences, and supporting them to exercise choice Exhibits excellent assessment, documentation, and care plan implementation skills, including budgeting Communicates effectively with exceptional negotiation and interpersonal skills Thrives in a fast-paced environment with excellent prioritisation, time management and organisational abilities Is a proactive problem-solver with a commitment to maintaining confidentiality Displays a passion for delivery of a restorative wellness model, which aims at improving the quality of life for older Australians Is seeking and committed to a positive team and workplace culture reflecting our vision, mission and values Demonstrates business acumen and computer skills, including MS Word, Excel, Outlook, Teams and database systems Has at least a primary dose of COVID-19 vaccine Has experience in the care sector and holds a tertiary qualification in aged care, health and/or allied health (remuneration will be commensurate with experience and qualification) Is willing to share an on-call roster for remote telephone support with other Care Managers (approximately once every 2 months) Holds a First Aid certificate (desirable) What's in it for you?
