You will be working collaboratively with clients and key stakeholders to provide comprehensive assessments and individualised case management. You will play a key role in promoting the client’s goals and wellness outcomes. What will this look like on a day to day basis? You will be: Conducting HCP assessments and reviews with clients and their families / representatives in accordance with the HCP guidelines; Actively seeking new clients to expand our client base and support business growth; Networking, attending industry events and generating leads as well as collaborating with Comlink’s marketing and customer engagement teams; Engaging with potential clients, and helping them to understand our products, services, and value proposition; Maintaining high quality assessment and care planning documentation; Fostering respectful relationships with clients to fully understand their needs and ensure our services are reflective of person centred practice and the Aged Care Quality Standards; Providing comprehensive case management, ensuring the day to day wellbeing of our clients through case work, advocacy and planning; Working collaboratively with other team members, community organisations, allied health providers and service agencies to ensure the best outcome for the client and their care; and Managing client feedback and complaints regarding service delivery in line with internal policies and ACQS requirements, undertaking remedy actions to address issues. Other things you might like to know: Some travel required (kilometre reimbursement offered); and Fully paid compliance checks offered, including NDIS Worker Screening. What skills and experience do you need to be a Care Partner? This position is best suited to someone who: Has a qualification in Community Services, Human Services, Health Sector Management, Aged Care and/or Clinical background (Minimum Diploma) and ideally a minimum of 3 years’ experience in the industry Is relatable and engaging with highly developed interpersonal skills, communication skills and emotional intelligence A proven ability to build strong relationships, connect with internal and external stakeholders and balance an outgoing nature with empathy and understanding Has a demonstrated understanding of the Aged Care reform, Aged Care Standards, the sector and people who are aging in the community Is experienced in case management preferably within the home and community care sector with advanced knowledge of person-centred care Has experience in service development, and driving continuous quality improvement and practice and service excellence aligning to Aged Care Quality standards How to apply Be creative!
