Case Manager - Enrolled or Registered Nurse Full time Glenview Community Services are a leading Tasmanian aged care and disability provider based in Hobart’s northern suburbs. As an AHPRA registered Case Manager, you will be at the forefront of our community care, providing high quality service and driving a range of options that support the goals of our clients in their pursuit to maintain their best life. You will work with team members to pursue and deliver excellence in clinical, financial, relationship and emotional support for our community clients. The successful candidate will hold degree qualifications in Nursing; AHPRA registration and a current practising certificate, with experience in case assessment in a clinical or community setting. In addition, you will demonstrate: Strong clinical care capabilities for an aged care and disability environment. Strong financial, organisation, prioritisation and problem-solving skills. Knowledge of the Home Care Package Program, CHSP and NDIS program requirements. High level influencing, collaboration and communication skills. High level service quality and continuous improvement capabilities. Change adaptability, flexibility and team orientation.
