About the Role We are seeking a values-driven and proactive Case Manager - Intensive Case Management Initiative! You will provide intensive case management and outreach to support clients. Reporting to the Team Leader, this is a permanent part-time position, 30.4 hours per week, based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Award Level 5. You Will Successfully Provide brief intervention, service coordination, case management, referral, and advocacy to successfully achieve client goals. Build connections with internal TSA Programs, services, and streams; builds and uses inter-agency and community connections and partnerships. Participate in team meetings and service planning workshops. Trauma-informed, strength-based, and client-centred practice principles are evidenced in all client supports. Promote therapeutic interventions, engagement with health and other treatment services and social inclusion activities. You Will Have Diploma or Degree qualification in Social Work, Psychology, or related field. Relevant experience in the social service sector including case management. Ability to demonstrate knowledge of homelessness including context, causes, models and approaches relating to best practice. Experience working with internal and external stakeholders to maintain and create new connections in the sector. A current and valid VIC Working with Children's Check. A current and valid Australian Driver's License.
