Why This Role Matters Journey to Social Inclusion is a case management service for individuals experiencing long-term homelessness in Melbourne, focusing on housing, quality of life, and community connection. It offers intensive, individualized support for up to three years, with a model that includes case management, housing access, trauma-informed practice, and skills development. Delivered by a consortium of organizations, it is funded by the Victorian Government’s Early Intervention Investment Fund. We are seeking a compassionate and motivated Case Manager to be involved in long-term case management and service coordination, ensuring high-quality, goal-oriented support for clients in line with TSA’s values and funding requirements. About The Role This is a permanent full-time position based in Geelong. Salary and conditions are in accordance with the Grade profile SCHADS Award level 5. Key Responsibilities Manage caseload and provide intensive support, service coordination, and advocacy Build and maintain professional relationships with clients using assertive engagement techniques Coordinate case conferences, support, and exit planning with referring agencies Apply Case Management and Trauma-Informed Care Frameworks Monitor program progress, outcomes, and maintain accurate client records Collaborate with ACCO and First Nation Workers for culturally safe support Commit to continuous quality improvement and TSA’s Strategic Plan Maintain links with TSA services and J2SI consortium partners Ensure alignment with national and state strategic plans and the National Homelessness Stream Model of Care Support J2SI research, build relationships with local agencies, and attend network meetings Implement continuous quality improvement (CQI) initiatives and enhance trauma-informed care practices. Essential About You: Appropriate degree qualification relevant to the delivery of community services e.g. Social Work, Psychology, Occupational Therapy or equivalent. Candidates with Diploma qualifications and outstanding relevant experience may be considered. Demonstrated experience in case management (anticipated likely 1 year of recent experience). Relevant experience working in the community service sector. Proven ability to restore relationships with people experiencing vulnerabilities and history of complexities in areas such as adult homelessness, alcohol and other drugs, mental health, disability, and family violence. Ability to work effectively in an outreach capacity. Sound knowledge of various theoretical frameworks and strategies applicable to working with clients with complex needs. Proven ability to effectively manage challenging behaviours and respond to crisis situations. Ability to participate collaboratively and constructively within teams of peers, staff, and other stakeholders, as well as initiative and effective personal judgement when working alone. Strong alignment to TSA Values A current and valid Working with Children’s Check A valid Victorian Driver's license. Desirable Experience in assisting clients to navigate different service systems to meet their needs. Knowledge of recovery principles and the ability to apply them in broader casework practice. Knowledge and skills to work assertively with clients. Specialist experience in a mental health and/or drug and alcohol setting.
