About the role Tweed Community & Allied Health have a vacancy for a Case Manager to join their Transitional Aged Care Program. This role provides case management, coordination of care, identification of required team members and discharge planning for clients on the Transitional Aged Care Program. Providing advanced clinical services, leadership and support as an integral part of the multidisciplinary health care team for patients/clients of the Northern NSW Local Health District (NNSWLHD).
