The Opportunity: This is a unique and rewarding opportunity to join the Ngaanyatjarra Council Group as a Integrated Chronic Disease Care Nurse delivering effective care coordination services for community members with chronic conditions, focusing on improving access to primary health care and promoting proactive disease management. Key responsibilities include building strong relationships with GPs and primary health services, raising awareness of the Integrated Chronic Disease Care Program (ICDC), and providing health literacy education to support self-management. The role also includes promoting MYHealth Record enrolment, developing and maintaining program policies and goals, ensuring timely completion of deliverables, and adhering to program guidelines. Other tasks involve documenting and reporting accurately, participating in relevant meetings, contributing to organizational objectives, maintaining compliance with occupational health and safety standards, and engaging in professional development. Additionally, the role requires supporting program evaluation, meeting contractual timeframes, and fulfilling other duties as assigned. About You: You bring extensive experience in remote Primary Health Care settings, with a strong background in Preventable Chronic Disease frameworks and management strategies. You have highly developed interpersonal and communication skills, enabling you to collaborate effectively with multi-disciplinary teams comprising individuals from culturally and linguistically diverse backgrounds. You have a deep understanding of issues affecting Aboriginal health, the principles of Primary Health Care, and the legislation governing work practices in this field. You excel at promoting and managing change in complex remote practice environments and are committed to Quality Improvement, evidence-based practice, research, education, and professional development. You are proficient in using a range of information technologies, including patient information and data management systems. Your qualifications include postgraduate studies in rural and remote health, or a relevant clinical discipline. Additionally, you have experience in care coordination, delivering chronic disease self-management training, and working in primary health settings. This is a FIFO role and will require you to travel between communities within the Ngaanyatjarra Lands; therefore, a current Manual WA Drivers License is essential.
