An exciting opportunity to join the South West Sydney region with Aspire4Life as a Clinical Aged Care Assessor . This is a community-based role allowing you the opportunity to travel throughout the region to work collaboratively with clients in their homes. We are open to both part-time and full-time permanent contracts working Monday to Friday (no weekend or shift work). This position offers the opportunity to promote independence throughout the clients' aged care journey. We offer a comprehensive 2 weeks of full-time training and orientation held both virtually and face-to-face in our Coolangatta office (all expenses covered). Utilising the integrated assessment tool with My Aged Care About you: Experience in assessment and goal-oriented support planning in aged, community, health or social services Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders Local knowledge and the ability to network with stakeholders within your region Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite) Previous experience working with KPI’s to ensure high-quality client outcomes Ability to manage your time effectively in an autonomous working environment What to bring? Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with AASW Registration Evidence of an influenza vaccination (for the 2024 Flu Season) Evidence of COVID-19 Vaccinations (minimum two doses + Booster) About Us: Aspire4Life acknowledges and pays respect to the Traditional Custodians of this land on which we work.
