About this role We have an exciting opportunity for a Clinical Care Coordinator to join our team at our Palms Aged Care Service, located in Ingham. As the Care Coordinator you will be responsible for providing advanced clinical support, leadership and coordination of clinical care for residents/clients with complex needs. You will contribute as an integral member of the Seniors Living group and supports activities aligned with the strategic direction of and consistent with the mission and values of Churches of Christ. In this role you will: Oversee and co-ordinate the completion of baseline and required focus assessments of individual residents/client's needs and record findings within a Positive Wellbeing Model Ensure that care plans are developed and reviewed based on the assessment findings of individual residents/client's needs, in accordance with the organisation's timelines Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument Monitor that resident/client documentation is accurately reflective of care provisions and is in accordance with organisational requirements Co-ordinate the care provision of external service providers Assist in the review and maintenance of clinical programs and initiatives Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines What you can bring to the team This role would be ideal for a Registered Nurse who is looking to relate to a new tight-knit community to advance their career.
