About ECHO ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence. We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP. Our goal is to provide services at the same time, on the same day and with the same team of Support Workers to ensure continuity of care and trust. Job Description The role: The Clinical Care Coordinator will bring a clinical focus to our person-centred care model, supporting elderly clients to achieve their goals and maintain independence. This role includes acting as a liaison with clients, families, caregivers, and health professionals while offering clinical insights and support to the team. Your responsibilities may include: Managing a caseload of home care packages, complex, and high needs clients Ensuring services meet internal and external quality standards Driving innovation and continuous improvement aligned with ECHO’s strategic objectives Offering guidance and support to clients, families, and team members to deliver safe, effective care Hours: Monday to Friday, 8:00 am to 4:00 pm. You will also be required to participate in a rotating on-call after hours, weekday and weekend roster. Desired Skills and Experience We are looking for someone who brings experience, compassion, and dedication to community-focused care. Registered Nurse with APRAH registration and a tertiary qualification in nursing Minimum 2 years experience in a case management role within aged care or community care Strong organizational skills and a proactive approach to problem-solving Excellent written and verbal communication, with a talent for building relationships and achieving positive outcomes Management of Home Care Package budgets Willingness to provide clinical care when needed Knowledge of Aged Care Quality Standards Understanding of Clinical Governance and Risk Management Make a difference with your clinical care skills – apply now and join our caring team! #J-18808-Ljbffr
