SUMMARY: Clinical Care Partner (Nurse) Home Care is required for a Not-For-Profit AMS located in Toowoomba. Help Close the Gap by providing Culturally Safe Services Up to 38hrs, Monday to Friday no after-hours and weekend work Full time preferred but will consider Part time for the right applicant Registered Nurse preferred but will consider an Enrolled Nurse for the right applicant Exceptional opportunity to assist vulnerable clients to have quality of life Organisation Overview Carbal Medical Services (Carbal) is a Not-for-Profit, charitable organisation that provides health services to the Aboriginal and Torres Strait Islander community in and around Toowoomba and Warwick. The core function of Carbal Medical Services is to provide medical services to Aboriginal and Torres Strait Islander people delivered through two medical practices and many community programs. Covering the Darling Downs and Southern Downs regions including Goondiwindi, this position will be based in Toowoomba and reports directly to the Service Delivery Manager. Position Overview The Clinical Care Partner will organise support for clients within CHSP (Commonwealth Home Support Program). The Care Partner identifies, implements and organises individualised support and care services in accordance with approved aged care funding, guidelines, legislative requirements, and organisational procedures. The Care Partner combines natural empathy and kindness with excellent communication skills to develop and maintain care plans for each client that incorporate the principles of person-centred care, dignity of risk, respect for individuality, autonomy and every older person’s right to choice. The Care Partner liaises closely with CHSP homecare support workers, as well as home maintenance contractors and external providers of services and equipment. Key duties will include but not be limited to: Conduct telephone consults and home care visits and regularly review care plans to ensure they meet the older person’s needs, goals and preferences concerning wellness, independence, reablement and social connections. Ensure care plans contain detailed, accurate and current information to enable workers to deliver care in line with each client’s specific needs, goals and preferences, including when circumstances change and ensure the client can access their care plan when and where they want it. Engage with clients respectfully and kindly and tailor communications to suit the needs of individuals, including those with hearing loss, visual or cognitive impairment, by setting out each client’s options (and what it means in practice for them) in a way the client can understand. Engage in practices and procedures to create an environment of person-centred care where clients are comfortable disclosing their identity, knowing they are valued and respected as autonomous individuals and being supported to live as they choose. Recognise and support clients' need to feel safe and in control by asking permission, offering choices, explaining what will happen, and working to build predictability, reliability, and consistency into relationships and services. Build the client’s skills and knowledge to fully understand and engage with care and service options and make informed decisions. Show appreciation for Indigenous culture and adapt your care approach accordingly, promoting a culturally safe and inclusive environment. Demonstrate reliability, confidence, and strong communication skills in all interactions with clients, families, and team members. Qualifications - Essential Current AHPRA Registration as a Registered Nurse; Or Enrolled Nurse Demonstrated ability to be proactive and work cohesively with Management to achieve patient outcomes; Demonstrated experience in working effectively with Aboriginal and Torres Strait Islander people and a clear understanding of Aboriginal and Torres Strait Islander culture, or the ability to rapidly acquire the same; Varied knowledge of chronic disease issues affecting Aboriginal and Torres Strait Islander people; You will have a demonstrated understanding of the health issues found in our Indigenous Communities; Demonstrated ability to effectively self-manage workflow and report on outcomes against the action plan; Current Open C Class Driver's License valid in Queensland; Satisfactory Police Check – no Serious/Criminal/Court Record, and ability to obtain a Blue Card. This will be conducted on initial appointment and on an annual basis in line with Carbal's compliance schedule; Current First aid and CPR certification; Previous coordination experience within a Health Care, Aged Care, or Community Service organisation.
