Job Description

Clinical Home Care Manager (RN/EEN) | Aged Care | Canberra Total Care Nursing is dedicated to enhancing the quality of life for elderly individuals in the ACT. We are committed to providing compassionate and individualised care to assist our clients in maintaining their independence and well-being within the comfort of their own homes. We are currently seeking a highly skilled and experienced Clinical Home Care Manager to join our growing team. The successful candidate will be responsible for providing and coordinating service delivery to eligible clients in their residences. The Home Care Manager will collaborate with the nursing/support team and the clients/families to effectively manage, develop, and monitor individualised care plans. Key Responsibilities: Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders. Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits. Collaborate effectively with internal stakeholders to ensure clients’ needs are met including quality, compliance and rostering assistance. Coordinate the provision of quality person centred care according to the client's needs, service/care plans and available funding in accordance with budget. Collaborate with clients, their families, and healthcare professionals to gather pertinent information essential to the assessment process. Develop and execute personalised care plans based on assessment findings. Conduct regular reviews and updates of care plans to reflect changes in clients' needs and preferences accurately. Coordinate and arrange a diverse range of in-home support services, encompassing personal care, nursing, therapy, and allied health services. Establish effective communication with external service providers to guarantee the seamless delivery of care. Act as an advocate for clients by ensuring that their rights, preferences, and choices are respected and upheld. Regularly oversee clients' progress and adjust care plans as necessary. Perform routine assessments to gauge the efficacy of the care provided and offer suggestions for enhancements. Maintain precise and current records of client assessments, care plans, and ongoing communications. Additionally, produce reports as necessary for both internal and external stakeholders. Prepare reports as needed for both internal and external stakeholders. Be prepared to provide skilled nursing in the community as required. Represent Total Care Nursing at external forums and meetings, promoting the organisation within the community. Qualifications and Experience: A current APHRA registration with no conditions or a related field is essential. Prior experience in case management within the aged care sector is required. A comprehensive understanding of the Australian aged care system and portals is essential. Exceptional communication, interpersonal, and problem-solving skills.
