About the facility Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health, and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. About the role The Complex Diabetes Clinic CMC will be responsible for the triaging and coordination of referrals and appointments for the Complex Diabetes Clinic. The CMC will provide coordination of the patients' care throughout their pregnancy with a focus on continuity of care. The CMC will liaise with patients, book appointments, provide midwifery appointments, and will be available to take phone calls and respond to messages. CMC Responsibilities: Includes working collaboratively with the MDT Team to ensure the appropriate review and follow-up of women going through the Complex Diabetes Clinics. This includes coordinating face-to-face reviews with Obstetricians, Endocrinologists, Diabetes Educators/Nurses, and allied health practitioners, as well as liaising with women and the wider multidisciplinary team in developing antenatal, intrapartum, and postnatal care plans. The CMC will assist and help facilitate the High Risk Complex Diabetes Clinic MDT, as well as assisting with auditing and data analysis for the clinic.
