Job Description: The Assertive Community Treatment Team (ACTT) at Fremantle Hospital Mental Health Service is based on the assertive community treatment model. This is a community psychiatric rehabilitation approach characterised by intensive contact with patients in their own environment to help them build/rebuild their lives. As part of a multidisciplinary team, the Clinical Nurse provides clinical and professional expertise to ensure comprehensive evidence-based nursing care is delivered to all patients. Liaising with other disciplines to provide a holistic approach within the Recovery Model. Facilitates and promotes patient safety and quality of care at all times. Working alongside patients, carers, and liaising with other appropriate services/agencies to optimise care and service provision. The Clinical Nurse practices within their scope of practice considerate of the Nursing and Midwifery Board’s Nursing Practice Decision Flowchart. This is a recruitment pool for permanent full time and fixed term full time appointments. Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 30/01/2026.
