The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Your role as the CNC Grade 2 will see you work in close liaison with staff from Primary Care and Community Health, Acute facilities, General Practitioners, Specialists, other healthcare providers (including private providers and Residential Aged Care Facilities), non-Government agencies and consumers and their carers and families to improve outcomes for people with a life limiting illness. You will work collaboratively with other specialist supportive and palliative care nursing positions (including the Nurse Practitioner, CNC3, CNC2 and CNS2) to ensure a whole of district approach to the provision of supportive and palliative care to the target client group. This will be achieved through leadership, clinical advice, consultation, and education and support of health professionals in the provision of evidence based supportive and palliative care management. What you will bring to the role Current Registered Nurse Division 1 with the Australian Health Regulation Practitioner Regulation Agency (AHPRA) with 5 years full time equivalent postgraduate experience and a minimum of 3 years full time equivalent experience in Supportive and Palliative Care nursing and post graduate qualifications relevant to the specialty area of practice or such other qualifications or experience deemed appropriate by the organisation. Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice. Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework. Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research. Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes. Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs. Demonstrated clinical expertise in client-centred consultancy with the ability to work independently. Current valid NSW drivers licence with proven good driving record and willingness to use for work purposes. This will include driving long distances across the Local Health District.
