About the Role Community Mental Health Services provide a range of services designed to help support the recovery and help consumers stay well in the community. The team provides specialist services for young people, children and families, adults, and older people. Community mental health services include a specialist Aboriginal Mental Health Team, bilingual mental health workers, and Peer Support Workers. Our main sites are at Canterbury, Croydon, Camperdown, Marrickville, and Redfern Community Health Services. The role will be based at Croydon. The Community Mental Health/Core Team model of care states:The “Community Mental Health Team” (Core Team) is the mainstay of the community mental health system. Core Teams provide a range of clinical services including intake, assessment, consultation, treatment, and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short-term care coordination. Croydon Core Team runs a DBT program for eligible consumers of the service. Consumers under the care of Core Teams will either have: Short-term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months. A period of treatment as recommended by evidence-based practices. Ongoing care and monitoring for prolonged periods. The Mental Health Nurse will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description. Ideal Candidate Current nursing registration with AHPRA. Demonstrated competencies to provide contemporary mental health practice. Current NSW Drivers Licence (P1 and P2 acceptable).
