As a Diabetes Educator, you will play a key role in providing personalized diabetes education, supporting patients with their daily diabetes management, and fostering a proactive approach to health and well-being. Key Responsibilities: Provide diabetes education and management support in patients’ homes, including lifestyle advice, medication management, and blood sugar monitoring. Develop individual care plans in collaboration with patients, their families, and the broader healthcare team. Educate patients about healthy eating, exercise, and the impact of diabetes on their overall health. Promote self-management skills and empower patients to take control of their condition. Offer emotional support and guidance to help patients manage the psychological aspects of living with diabetes. Maintain accurate records of patient progress and communicate effectively with the healthcare team. Qualifications & Skills: Registered Nurse with current AHPRA registration. Certificate in Diabetes Education or equivalent. Experience in diabetes management and education is preferred. Strong communication and interpersonal skills, with the ability to work independently and as part of a team. A compassionate, patient-centered approach to care. Ability to develop trusting relationships with patients and their families.
