Epworth at Home Hospital in the Home Coordinator Full-time Epworth HealthCare is Victoria’s largest not-for-profit private health care group, renowned for excellence in diagnosis, treatment, care and rehabilitation. This new Coordinator role is an exciting opportunity to join the expanding Epworth at Home service. The focus of this full-time position is the patient journey into the Hospital in the Home and Chemotherapy in the Home areas of Epworth at Home. This position will provide clinical and professional leadership across the Epworth at Home team, and broader Epworth services, whilst collaborating with key stakeholders to develop strong relationships. This role also encompasses the revenue component of HITH and CITH, with a strong knowledge of Health Funds required. Epworth Hospital in the Home was the first private sector HITH unit to be established in Australia, and has been in operation since 2000. Currently, the unit treats over 700 acute patient admissions and over 7500 bed days each year, with a planned expansion in progress. The future of acute healthcare will involve the delivery of more technical hospital level interventions at home, for conditions that would otherwise require traditional hospital interventions and care. To be successful in this role, applicants will be registered to practice nursing in Victoria with AHPRA, and have a recent background in hospital care. A driver's licence is necessary. Prior experience in Hospital in the Home and/or Chemotherapy nursing is essential. To be successful in this role you will: Have general clinical nursing experience across different nursing backgrounds Be committed to the mission of delivering high quality acute hospital care to patients at home, including service development and evaluation Ability to market and liaise with physicians and multidisciplinary teams Previous management experience and/or qualifications (desirable) Background in CCC experience (desirable) Exhibit teamwork and compassion capabilities Be flexible, willing to learn and accept challenges Have a “go-getter” attitude and an approachable personality Have excellent leadership and communication skills Demonstrate strong problem-solving capabilities, organisational skills and time management About Rehabilitation & Mental Health Epworth Rehabilitation and Mental Health services are some of the most progressive of their kind in Australia.
