As the GPACI Project Officer, you will use your ability to communicate information and foster stakeholder relationships with staff in Residential Aged Care Homes (RACHs) to support the implementation of the General Practice Aged Care Incentive Program. You will influence change for improving the quality of primary health care service delivery to RACH residents. You’ll make an impact at scale by: Service and program implementation. Supporting and facilitating positive change. Data management. What you'll bring To succeed in this role, you will have: Demonstrated skills and/or experience in working in health or RACHs. Ability to build, develop and sustain positive partnerships and productive working relationships with all levels of stakeholders (internally and externally), with a commitment to customer service. Ability to communicate information to staff in a residential aged care setting, with the skills to influence change for improvement in the quality of primary health care service delivery to RACH residents. Ability to work autonomously and as part of a team. Excellent computer skills (including word processing, data entry and use of a range of software applications). Why South Western Sydney PHN?
