Matthew Flinders Care Services is located in the beautiful Port Lincoln on the lower west coast of Eyre Peninsula of South Australia, known as the seafood capital of Australia. Port Lincoln is a thriving rural community surrounded by beautiful coastline. Local attractions include beautiful beaches and national parks offering an array of outdoor activity, ocean experiences and local seafood dining experiences. About the role We are seeking to employ an enthusiastic Case Manager to join the Home Care team. As a Home Care Case Manager, you will be working closely with our Care Recipients to understand their goals and to help them live their best lives in their own homes. Daily tasks may include undertaking assessments, conducting quarterly or annual reviews, developing support plans, working with the Care Recipient to implement new services, arranging equipment and helping Care Recipients to make decisions to best utilise their Home Care Package. The role is ideally Monday to Friday 8:30 - 4:30pm. Benefits and perks Full-time or part-time positions, flexible work arrangements to support work-life balance. Skills and experience required to be successful in this role Nursing registration preferred but not essential. Experience in Aged Care desirable. Exceptional Care Recipient case management and time management skills. Ability to work in a diverse team environment with flexibility to meet the needs of our organisation. Knowledge of Home Care Packages highly desirable. Excellent communication and clinical problem-solving skills. Excellent computer and documentation skills. Empathy and an emphasis on treating others with dignity and respect. The right to work in Australia. Have a current NDIS Worker Screening or be willing to obtain. Pass a pre-employment medical. Provide evidence of current Influenza and Covid-19 vaccinations. Come and join us!
