Work with a team to give every consumer the best day possible through the delivery of lifestyle and wellness activities that support the individual preferences and needs of each consumer. Develop, plan, deliver and evaluate services in line with the consumers individual wishes in a sensitive, dignified and appropriate manner promoting independence and a meaningful life. Key Responsibilities: In collaboration with consumers plan, develop, implement and evaluate meaningful activities within and outside the facility that are respectful to the rights of all consumers. Identify individual consumer lifestyle goals and needs and provide support to achieve these giving consumers control and choice over their lifestyle. Ensure the needs and preferences of consumers are met effectively and appropriately and report changes to needs to the clinical team. Assist consumers to live in a homelike environment whilst ensuring the safety and dignity of everyone at the facility is maintained. Support consumers to maintain independence and decision making, at times enabling them to take assessed risks. Communicate effectively with consumers and their representative to ensure effective relationships and support them to have the opportunity for feedback. Supporting care staff as needed in basic ADL and manual handling tasks, including assisting residents with toileting, to eat and dress; pushing residents in wheelchairs and fall out chairs; providing stand-by assist mobilising and transferring in/out of buses, pushing and manoeuvring of trolleys and assisting residents in emergency evacuation situations. Providing one-on-one care (where required) to assist residents to take part in social activities, or provide one-on-one social support. Contribute to the general housekeeping as required for each location. At all times work within the facilities policies and procedures to ensure safety and identify any areas of improvement required within these. Maintain confidentiality and privacy of all consumers and ensure respect and dignity at all times. Ensure documentation is completed in required format, in required timeframes and that advice is sought when required. Contribute to a positive team and organisational culture. Take direction and guidance from more senior staff members in a positive manner. Where medication competence is part of the assigned role, ensure medication is managed safely and correctly. Work effectively as part of the multidisciplinary team to achieve the best outcomes for all residents in an inclusive and respectful environment. Mentor and supervise volunteers in their roles associated with the lifestyle team. Undertake manual handling tasks as required. Staff must have the capacity to undertake manual handling tasks with residents (of up to 130kg) utilising provided equipment. Experience and Qualifications Required: Minimum of Certificate 3 Individual Support (Aging) together with demonstrated relevant experience in delivering Lifestyle activities. Eligibility to work within Australia. Current Driver’s Licence. Current National Police Clearance Certificate. Knowledge of dementia, ageing and wellness and the role of provision of meaningful activities for residents. Previous experience in planning and delivering creative and innovative Lifestyle and wellness services. Moderate level of fitness to carry out duties. Demonstrated ability to manage competing priorities, professionally and as part of a team. Demonstrated interpersonal skills. Knowledge of Work, Health & Safety Legislation. Proven experience providing empathy for the elderly and disabled. Previous experience recording and maintaining electronic clinical records relating to patient and general computer skills.
