Manager Residential in Reach | Permanent Full Time opportunity including a monthly ADO About The Role The Residential In Reach (RIR) Manager plays a crucial role in enhancing the delivery of care to elderly residents within Residential Aged Care Facilities (RACFs). In collaboration with multidisciplinary teams, this role focuses on providing short-term, person-centred nursing care to prevent unnecessary hospital admissions. You will have the opportunity to lead service development projects, ensure quality care, and contribute to improving health outcomes for the elderly. Key Responsibilities Lead and manage the RIR service, focusing on service development and quality care. Provide consultative support and case management to prevent emergency hospital admissions. Collaborate with RACF staff, GPs, and Grampians Health medical teams. Develop and implement telehealth services to promote timely interventions. Provide high-level clinical support and education to RACF staff. Manage and monitor data, financial budgets, and resources for the RIR service. Skills & Experience Eligible for AHPRA registration as a Registered Nurse with current practicing certificate. Minimum of five years’ post graduate experience. Advanced clinical skills and the ability to work in multidisciplinary teams. Strong communication and negotiation skills. Ability to assess, plan, coordinate, and evaluate health programs.
