Medical Coordinator/Direct Support Professional Job Category : Direct Service Worker Requisition Number : MEDIC003799 Apply now Posted: February 1, 2025 Full-Time On-site Rate: $19 USD per hour Locations Showing 1 location Responsibilities In collaboration with the RN, ensure that all medical appointments are scheduled in a timely manner and in accordance with regulations. Prepare clinical consultation forms for all medical appointments. Ensure sufficient staff scheduling for each appointment in collaboration with the RN and the Residence Manager. Accompany individuals and/or staff on medical appointments whenever possible. Review and file all medical documentation signed by the RN and/or Residence Manager. Maintain up-to-date medical appointments and lab work records for all individuals. Document and implement all physician recommendations. Develop weekly/monthly medical appointment calendar as determined by the RN and Residence Manager. Maintain AMAP certification. Submit prescriptions to ChemRx Pharmacy upon receipt and ensure proper counting/recording of medications upon delivery. Complete the “Client-Specific Sheet” for new medications delivered. Check Medication Cart/Room weekly for expired medications and sufficient supply for a minimum of five (5) days. Reorder medications when the supply is insufficient. Ensure ChemRx Pharmacy receives all original prescriptions. Review and log all controlled substances weekly to ensure compliance. Discontinue medications as indicated by the prescribing physician in collaboration with the RN. Maintain a clean and orderly Medication Room/Cart and order medical supplies as needed. Ensure all AMAP signatures are on the Medication Administration Record (MAR) prior to the first administration each month; file MAR documentation monthly. Prepare off-site medication for home visits, trips, etc., in accordance with OPWDD regulations and agency policy. Assist RN in maintaining individuals’ medical binders and completing various forms (infection control, medical appointment, nursing report, resident report). Complete all paperwork as requested by RN and Resident Manager. Schedule and complete quarterly pharmacy reviews. Monitor Bowel Movement charts daily and report any issues to RN and Manager. Perform additional duties as requested by the RN, Residence Manager/Assistant Residence Manager, Program Coordinator, or Program Director. Assist with morning routines and daily living activities (ADL outcomes, making beds, breakfast clean-up, etc.). Assist with dinner preparation if medical coordinator responsibilities are completed. Support individuals after programs with lunch preparation and other requests. Complete laundry and food shopping if time allows and medical coordinator responsibilities are fulfilled. Qualifications Comprehensive knowledge of OPWDD regulations. Strong organizational and interpersonal skills. Good oral and written communication skills. Valid NYS Driver’s License.
