About the role The Mental Health and Housing Liaison Officer (MH&HLO) will work with Homes NSW to link people who require mental health services who are homeless, or at risk of homelessness, to housing services and help them access and/or sustain tenancies. Improving the connection, pathways and collaboration between Mental Health Services and Homes NSW to achieve better outcomes for consumers in accessing and/or sustaining tenancies. Utilising their expertise in mental health and housing models and pathways to support complex care coordination to ensure community tenure and: access public and social housing maintain tenancies through support packages such as the Housing and Accommodation Support Initiative (HASI) and Community Living Supports (CLS) program. Liaising with key Mental Health teams, Housing teams, and community managed organisations (CMOs), project management, and consultancy. Forming part of local Housing and Mental Health 2022 (HMHA 22) committees, which will be an important touch point for Health, Housing and other partner agencies in the district. The role will be mobile throughout the district, including provision of in-reach to inpatient Services and co-location at housing offices.
