We are seeking experienced professionals in the fields of social work, mental health, community services, counselling, welfare, behaviour management, case management, forensic services, or homeless services to join our dynamic multidisciplinary team. Key Responsibilities: Comprehensive Recovery Case Management : Develop and manage tailored recovery plans for residents using the Recovery Star outcome measurement tool. Triage and Referrals : Provide support with triage, panel, and referral processes. Relationship Building : Foster strong partnerships with state-wide mental health and justice services. Risk Management : Perform risk assessments, manage risks, and follow up on incidents. Trauma-Informed Practices : Assess and develop trauma-informed services to enhance resident outcomes. Documentation : Maintain accurate, up-to-date documentation and reporting. Group Facilitation : Lead therapeutic group sessions and activities. Individual Support : Provide one-on-one key worker support and maintain a caseload of residents. NDIS Coordination : Help residents navigate the NDIS and achieve comprehensive packages. De-escalation and Crisis Intervention : Implement and support effective behaviour management strategies and respond to behaviours that challenge. Job Description Woodville House is a 24/7 intensive supported accommodation pilot service designed to support individuals experiencing chronic homelessness, with significant co-morbidities, trauma, and medical complexities. We take a holistic, trauma-informed, and person-centred approach that aims to foster stability, recovery, and improved health outcomes. About You To be considered for this role, you will possess: Qualifications and Experience: Relevant qualifications in social work, mental health, or related fields. Experience in homelessness support, mental health, and trauma-informed care. Well maintained professional self-care, self-leadership and resilience. Commitment to evidence-informed practice and continuous improvement. Ability to work well within a diverse and adaptive MDT. Highly Desirable: Experience working with FDV. Experience working with AOD. Experience responding to behaviours of concern.
