The Role: The NDIS Service Coordinator will support a small caseload of individuals, either in their own homes or in a supported independent living (SIL) home. The NDIS Service Coordinator provides leadership to a team of support workers responsible for maintaining an environment that empowers people with a disability. Reporting to the Client Service Manager, you will be responsible for: Ensuring support to a small caseload of individual clients in their homes is delivered in a manner consistent with the values of InLife Independent Living Mentoring and providing leadership on a day-to-day basis to the team Liaising with and developing relationships with external stakeholders including clients' families, allied health professionals, and support coordinators Data reporting in line with individual plans Day-to-day financial management Client health and wellbeing tracking, ensuring follow-up and individual appointments Supporting clients to work towards goals Using Workplace to share and communicate information for each client team Ensuring the team maintains all required reporting Managing/maintaining the physical premises and associated activities Undertaking shifts doing direct support work as per client plans and in consultation with your manager Holding regular client team meetings according to client needs The Person: You’ll be driven and show genuine passion for building capability within your team. In addition, you: are a natural influencer; modeling a positive attitude, professional conduct, and unwavering commitment to quality outcomes and service delivery for staff, individuals with disabilities, and the community enjoy working with a high-performing group of colleagues and peers and accept responsibility and accountability are tech-savvy and comfortable with using and/or learning our cloud-based apps are self-motivated, well-organized, and resourceful possess a high level of integrity, passion, and reliability are an advocate for independence have a positive track record supporting people with complex presentations including intellectual disabilities, complex health, and physical disabilities possess relevant qualifications and/or experience in Community Services/Case Management (or equivalent) are experienced in leading a team and supervising the work of others hold a current Victorian Driver’s License The Offer: The NDIS Service Coordinator position is offered on a permanent basis at a competitive salary plus superannuation.
