The role is responsible for supporting the Continuing Care Team (CCT) and managing Care Awaiting Placement (CAP) beds co-located at a local Residential Aged Care Facility. The Continuing Care Team is responsible for managing the placement of consumers within our NALHN Hospitals who require Residential Aged Care. The main focus of this role is line management for the staff within CCT, coordination and leadership of the team, and ensuring the team continues to achieve high quality patient outcomes. Management of the Care Awaiting Placement beds will require you to build close working relationships with external providers, an ability to negotiate and be flexible, and to maximize efficiencies in hospital flow. To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.
