Job Description

We are looking for a Nurse Practitioner (NP) – Cardiology to join our Chronic Conditions Service. In this role, you will work within a collaborative model of care to deliver safe, good quality care and treatment to the patients of the Chronic Conditions Service. This is a permanent full-time opportunity. The Nurse Practitioner (NP) – Cardiac Rehabilitation/Cardiology is a Registered Nurse who demonstrates advanced and expanded clinical skills, knowledge, and a high level of capacity to translate these into contemporary evidence-based clinical care for complex post-acute cardiology patients. Within a credentialed scope of practice, the Nurse Practitioner provides nursing leadership, clinical governance, complex care coordination, and support for patients of the Chronic Conditions Service. About you Our ideal candidate will be someone who can demonstrate the following: Demonstrated relevant clinical expertise to the level required within the Mandatory Qualifications section. Demonstrated skills and application of data analysis; outcome evaluation; service improvement and development through Quality, Safety, and Governance activities. Demonstrated advanced and effective interpersonal and written communication skills to enable accurate, complete, and timely assessment, diagnosis, determination, and information transfer to patients, their family/carers, nurses, or health professionals team members to support patient-centred care and staff's professional development. Demonstrated knowledge of legislation and policy relevant to the position and the ability to develop documents supporting the application of the same within healthcare workplaces. Leadership proficiency through the articulation of team engagement principles and personal attributes that influence a positive team culture and achieve active collaboration across multidisciplinary services. Proficiency in the establishment of therapeutic relationships, incorporating health education, advocacy and culturally competent professional practices that facilitate achieving patient identified goals and optimised health experiences. Applied knowledge of quality improvement frameworks and associated quality processes inclusive of the integration of nursing research, identifying experiences that evidence a professional commitment to contributing to quality improvement. Mandatory Requirements: Registration as a Registered Nurse with Endorsement as a Nurse Practitioner with AHPRA and a current annual practising certificate are mandatory for this position.
