Nurse Practitioner - Orthopaedic IMPACT - (Infectious Disease/Imaging Microbiology Arthroplasty Complex Trauma) Orthopaedic Outpatient Department are looking for Nurse Practitioner – Orthopaedic IMPACT – (Infectious Disease/Imaging Microbiology Arthroplasty Complex Trauma) to join their team. The Nurse Practitioner (NP) will provide and coordinate comprehensive care of patients using advanced practice skills in Orthopaedic Surgery. About the Role The role of the Nurse Practitioner Orthopaedic IMPACT will: Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment, and referral as required. Work in collaboration with the IMPACT service targeting patients that are referred following complex trauma. This will include a team of clinicians from Infectious Disease Microbiology, Plastic & Reconstructive Surgery, Arthroplasty and Complex Trauma specialists. Provide coordination of the IMPACT Multidisciplinary Case Conference (MDCC). Provide safe, quality, cost effective care and practice within the Nurse Practitioner's scope of clinical practice, that is consistent with contemporary standards and evidence-based practice. Implement and lead nurse led models of care within the Orthopaedic IMPACT team to support the complex needs of these patients. Work collaboratively with the health care team and contribute to enhanced patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to evolving patient and service needs. Provide referrals to appropriate agencies, including primary health settings. Provide coordinated patient centred care, creating partnerships, improving patient outcomes and facilitating systems improvement. Improve the delivery of complex patient care, increase system efficiencies, and reduce costs by streamlining and coordinating care. Lead and review processes that support an efficient patient care journey through knowledge of systems and processes enhancing care integration and reducing fragmentation. Integrate and lead education, research, and data management processes to support best practice and evidence-based care, planning and evaluation.
