Work in an environment that offers: Support for career progression and development Services to support people within their home A dynamic and growing health service that embraces new technologies The Position Job Reference: 1792 Position Title: Nurse Unit Manager - Bendigo Health @ Home (HITH, PAC, HARP & Diabetes Education)Classification: NM12Salary per fortnight (exc. super): $5,270.60Hours per fortnight: 80 (including an ADO every 4 weeks) Employment Status: Ongoing Full Time Role Description The NUM Bendigo Health @ Home will be responsible for effective leadership, management, direction, support and development of staff within the Hospital in the Home, Post-Acute Care, Hospital Admission Risk Program and Diabetes Education programs: Hospital in the Home (HITH) provides individualised and holistic care in a patient’s home that would otherwise be provided in an acute hospital setting Hospital Admission Risk Program (HARP) provides integrated and coordinated care to clients with a chronic disease, aged and/or complex needs who frequently use hospitals or are at risk of hospitalisation Post-Acute Care (PAC) provides service to clients to support their transition from hospital to home Diabetes Education works as part of the wider Endocrine and Diabetes team within Bendigo Health The Manager will: Provide day-to-day operational leadership for Bendigo Health @ Home program including the streams of HITH, PAC, HARP and DE Maintain and enhance standards of clinical excellence that reflect the vision and goals of Bendigo Health Develop a comprehensive knowledge of current trends and developments in the health industry that relate to HITH, PAC, HARP and DE services. Operate within the framework of NSQHS, best practice and quality improvement. Work collaboratively with senior management and colleagues across Bendigo Health to ensure efficient and effective strategic and operational performance, planning and service delivery, including maximising occupancy of programs and timely response to referrals Contribute to patient flow initiatives to assist with Bendigo Health bed management Ensure Bendigo Health @ Home is actively represented at relevant internal and external forums/committees Ensure staff have access to appropriate educational opportunities, and that performance reviews are conducted annually with all staff Promote the observance of accepted ethical and professional behaviour amongst all staff in line with Bendigo Health values In conjunction with AH&CC staff provide strategic direction and review progress of the strategic plan Complete the financial management of the department by monitoring the financial performance and undertaking analysis of performance of services/activity and making recommendations as required Provide high quality verbal and written reports, presentations or submissions as required Working with Bendigo Health Bendigo Health are a leader in regional health care, combining outstanding services with a strong education, training and personal development culture.
