What you'll be doing As Australia’s population ages and the number of people using aged care services increases, it is important that older people within residential care are supported to receive high quality care, including access to specialist palliative care. The Residential Aged Care Transitional Nurse Practitioner/Nurse Practitioner (TNP/NP) provides readily available access to high level clinical expertise and support to clients within residential aged care facilities across Western Sydney Local Health District (WSLHD). The TNP/NP is an expert clinician who manages and coordinates episodes of care as the primary provider or as part of the multidisciplinary team. The TNP/NP functions as an expert nurse clinician with extensive experience in care of the older person and palliative care. The NP/TNP is authorised to function autonomously in collaboration with a WSLHD Specialist Supportive and Palliative Care Physician/s (NP), or WSLHD Geriatrician; or under the supervision of a WSLHD Specialist Supportive and Palliative Care Physician/s (TNP) or WSLHD Geriatrician in an advanced and extended role which exceeds the scope of practice of a Registered Nurse (RN) or Clinical Nurse Specialist (CNS). The TNP/NP works together with GPs and RACF staff to ensure resident’s goals of care/ advance care plans are agreed upon and upheld. The TNP/NP provides evidence-based autonomous clinical care for clients within approved, established guidelines for advanced nursing practice and understands their accountability and works with the wider care team to secure the most appropriate care for the clients. In addition, the TNP/NP will have the required knowledge, skills and attributes to utilise the latest evidence to inform practice, deliver and coordinate care. Essential Criteria: Qualification: Post graduate qualification; Masters of Nursing (Nurse Practitioner) or enrolled and/or working towards and will be completed within 4 years. Registration: Australian Health Practitioner Registration Agency (AHPRA) as a Registered Nurse (TNP) or Registered Nurse Practitioner (NP). Selection Criteria: Demonstrated evidence of post graduate qualification; Masters of Nursing (Nurse Practitioner) or enrolled and/or working towards and will be completed within 4 years. Demonstrated high level of knowledge and advanced clinical care skills in care of the older person and palliative care, including comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to a range of challenging complex, changing environments. Extensive experience in palliative care and a demonstrated competence in appropriate use and prescribing of opiates and other EOL medication. Proven high level knowledge and expertise in complex client centred consultancy, practice development activities including mentoring education, policy development and quality improvement. Demonstrated ability to work with a high degree of autonomy and accountability while also working collaboratively in a multidisciplinary and multi service environment. Demonstrated advanced communication and interpersonal skills across a variety of clinical settings and proven high level of self-efficacy. Demonstrated commitment to ongoing professional and educational development of others, with a commitment to own professional development and currency of practice. Current Drivers Licence valid within Australia and willingness to use for work purposes.
