About The Role HammondCare regards this role as key in the delivery of relationship-based care which seeks to understand and value every individual as a whole person. Reporting to the Pastoral Care Manager you will utilise and share your pastoral expertise, coaching staff, and volunteers to support the emotional and spiritual needs and goals of clients, their families, and staff. This is a permanent part-time role (flexibility of 2 or 3 days per week) based at our residential site at Cardiff. Act as a role model for staff and as an advocate for HammondCare’s Mission, Motivation and Mission in Action, and its Christian values Share pastoral expertise and coach staff to support the spiritual needs of residents Conduct an assessment of resident/family needs and goals on entry to the service using the HOPE assessment tool Develop a spiritual care plan, working with residential staff to ensure client/family spiritual needs and goals are acknowledged As required, participate in case management reviews and make a contribution about the client’s emotional/spiritual needs and goals and contribute to the development and ongoing evaluation of a care plan Lead regular carer support groups Network with other religious leaders and services and coordinate relevant care, resources and group activities Strategically develop tailored resources, ideas and activities to enhance the spiritual and emotional wellbeing of residents and families, collaborating with management, staff and volunteers Communicate and build trust by developing relationships with staff and volunteers. Liaise with managers, case managers and attend staff and case management meetings as required About You Strong understanding and experience of people living with dementia, the impact on carers, and journeys of grief and bereavement Strong personal and active Christian faith and church involvement Theological /Pastoral Care qualifications and experience Demonstrated capacity to pastorally care for and enhance the wellbeing of those in need Highly developed communication skills including listening, compassion and empathy Experience raising up, leading and supporting a team Strong administrative and computer skills Our Culture HammondCare is committed to creating an inclusive workplace that promotes and values diversity and inclusion.
