Add expected salary to your profile for insights The Youth Initiative (YI) is a new initiative that aims to prevent homelessness among young people leaving care by building their capacity, skills and capability. The primary purpose of this position is to work alongside young people transitioning out of Out of Home Care (OOHC) services to support and mentor them to develop networks and skills for independence and successful community living. About the role: Foundations Care is seeking a Personal Advisor to develop a mentor-like relationship with PYI young people. This role will be based at our Tamworth, New South Wales location and is offered on a full-time fixed term contract basis until 30 June 2026. The Personal Advisor will be the central point of coordination and mentorship for all service elements accessed by the young person to support them to gradually develop the skills and strategies to manage change and crisis, build on the young person’s pro-social networks and support the facilitation and implementation of the young person’s leaving care plan. About you: You will ideally be an enthusiastic, knowledgeable individual with a heart for working alongside young people to assist them to develop the necessary skill to transition into independence. You will show flexibility in your approach with a focus on creating a trusting relationship with young person/s but with a clear goal of achieving planned outcomes aligned with the goals highlighted in the young person’s leaving care plan. You will demonstrate cultural competence in your approach and interactions at all times and will adhere to Foundations Care Ltd core practice and policy frameworks, engaging in self-learning or training opportunities to further your knowledge and skills. You will bring your knowledge and networking skills to the role and will demonstrate an ability to focus solely on building resources and strategies to improve positive outcomes for young people in their communities. You will have a minimum of a Certificate IV in related fields of Human and/or Community Services; be enrolled in; or be willing to obtain.
