This includes engaging residents in a range of diverse social, recreational, and cultural activities that are tailored to residents’ individual needs, goals, and preferences. This position plays an integral part in the culture of our Home and the health and wellbeing of each one of our residents, including staff. This position has casual and part-time shifts to offer. As the PCW (Lifestyle), you will ensure the delivery of person-centred care and services to all residents in activities of daily living in accordance with the residents’ choices, needs, and preferences. Reporting directly to the Lifestyle Coordinator and Registered Nurse In Charge, you will be responsible for: Provide individualised care including hygiene, grooming, manicuring, oral, and continence care. This includes assisting with planning, implementing social, cultural, and emotional activities designed to meet the individual needs of residents. Communicate with residents in a respectful and dignified manner, including assisting residents to achieve maximum independence, maintain friendships, and participate within the community. Develop and maintain interaction with residents and their families to ensure the provision of person-centered care. Assisting residents with behaviours of concern, by participating in the development and implementation of an individually designed management plan. Assisting residents with mobility, including walking, use of wheelchairs, mobility aids, and transfers. Supports and assists residents with nutrition and hydration requirements. Assist the nursing staff with treatments as directed. Assists the nursing staff by observing and reporting promptly any changes in the condition or wellbeing of residents. Ensure the residents' privacy, dignity, and respect are always maintained. This includes supporting and ensuring residents' individual interests, beliefs, and cultural needs and any diversity requirements are valued, supported, and respected. Ensure that no resident experiences isolation and loneliness unless determined by choice. Perform other duties within the scope of practice and as directed. Reporting in progress notes and providing relevant information to assist the RN/EN to carry out wholistic assessment and care planning. Complete incident reports where you have been a witness to. Assist with bed making and tidying work areas in a manner as directed by the consumer or as directed. Visual inspection of consumer’s room environment ensuring adequate supplies, safety, and cleanliness. What you will bring Minimum Certificate 3 in Aged Care Current first aid certificate High level of written and verbal communication skills Medication Administration evidence of a Nationally recognised training course where required for administration of medications Previous experience working in residential aged care. And where required, Medication Administration experience (good to have).
