Preparing in-home care assessments and care plans to best meet client requirements. Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks. Qualifications: Current registration with AHPRA Community nursing experience is a must Satisfactory Australian National Police check Working With Children’s Check Current first aid and CPR certificate Driver's licence and reliable presentable car Preferred Skills: The ability to operate efficiently and deliver excellent service Time management, organisational skills & ability to manage competing priorities Relationship development skills to support diverse clients to achieve their goals and objectives Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS) Responsibilities include: Care Plans and Assessments Government-Compliant Care Management Plans Holistic outcomes driven Care Planning Operation of the Care Planning and other management systems Personal care supports Quality assurance #J-18808-Ljbffr
