About the Role Our multi-disciplinary nursing team deliver the Advanced Preventative Care Program (APC) and Private Postnatal Service across Tasmania. As our business continues to grow, we are seeking to bring experienced Casual Registered Nurse Care Managers to join our (APC) team. Key Responsibilities Deliver high-quality nursing care to patients in the community, including assessments, care planning, and ongoing management of preventative care. Provide clinical expertise and support to individuals with chronic conditions. Assess and identify deterioration and changes in client’s conditions, communicating results of assessment to client’s medical practitioner and others as appropriate to individual needs. Work collaboratively with healthcare professionals, families, and carers to develop and implement individualised care plans. Provide education, advice, and support to patients and their families on managing health conditions and promoting wellness and goal setting. Ensure accurate and timely documentation of care and clinical activities. Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA). Minimum 5 years’ industry experience in acute care nursing with sound clinical assessment experience. Strong clinical and critical thinking skills with a focus on delivering person-centered care. Ability to work independently, manage a caseload, and use initiative. Demonstrated knowledge and experience in client assessment, care planning and experience in chronic disease management. Demonstrated ability in maintaining high clinical standards aligned with compliance, governance and the National Safety & Quality Health Service Standards (NSQHSS). Excellent communication skills, both written and verbal, with the ability to build rapport with patients, families, and colleagues. Proficient in the ability to use MS Office software suite. Must be willing to travel Launceston and as required regional areas within Northern Tasmania. Demonstrated commitment to ongoing professional development. A satisfactory National Police Check.
