Providing safe, compassionate, person-centred care through the assessment, risk assessment, care-planning, implementation, and evaluation of nursing care in collaboration with individuals and the multidisciplinary team to achieve safe quality care outcomes. Effectively planning safe transfer of care plans in collaboration with the consumer, their family or carers, the multidisciplinary team, community mental health team (CMHT), non-government providers, and other agencies.
