We are seeking an experienced Senior Community Care Manager to join our Community Services team and play a key leadership role in case management, service coordination, and process improvement. About the Role As a Senior Community Care Manager, you will oversee complex client cases, mentor care staff, and drive improvements in aged care services. Your leadership will support the coordination of care services, ensuring high standards of quality, compliance, and client satisfaction. Key Responsibilities: Complex Care Management & Client Support Lead and manage complex client cases, including dementia care. Provide expert guidance on care planning, service delivery, and external referrals. Conduct risk assessments and ensure appropriate interventions. Maintain compliance through accurate documentation and reporting. Leadership & Team Support Mentor and support Community Care Managers and Coordinators. Promote a culture of learning, collaboration, and continuous improvement. Facilitate case conferences and staff debriefing sessions. Process Improvement & Compliance Implement process enhancements to improve efficiency and service quality. Align service delivery with audit recommendations and regulatory requirements. Develop and maintain policies to ensure industry best practices. Stakeholder Engagement & Advocacy Build strong relationships with clients, families, and healthcare providers. Serve as a key liaison for high-risk cases, ensuring clear resolution strategies. Advocate for client access to appropriate services and supports. Business Efficiency & Growth Contribute to strategic planning for service improvement and sustainability. Identify opportunities for service expansion and business growth. Support the transition to the new Support at Home program. About You: Tertiary qualification in Social Work, Allied Health, Nursing, or related field. Extensive experience in aged care, disability services, or community case management. Strong knowledge of aged care programs, compliance frameworks, and risk management. Proven leadership skills to mentor, motivate, and develop high-performing teams. High-level problem-solving, conflict resolution, and decision-making abilities. Excellent communication and stakeholder engagement skills. Strong documentation, reporting, and case management expertise. Current National Police Check and Driver’s License. Why Join Bene Aged Care?
