You will work across multiple funding schemes, including NIISQ, icare, TAC, DVA, and other insurance providers. Your responsibilities will include: Conducting comprehensive client assessments to determine rehabilitation needs Developing and coordinating tailored rehabilitation and support plans Guiding and empowering clients throughout their recovery journey Liaising with essential stakeholders, including medical professionals, allied health providers, and support workers Advocating for client needs within available funding and service frameworks Collaborating with multidisciplinary teams to provide holistic and effective support Navigating complex systems and coordinating services to ensure seamless rehabilitation processes Utilising technology to facilitate remote work and efficient service delivery What You Need to Succeed To be successful in this role, you’ll need: A 4-year Degree in Social Work (preferred) or other allied health Active membership with AASW (for Social Workers) or AHPRA (for other allied health) At least 2-5 years of clinical experience in case management, rehabilitation, or disability services Excellent communication and interpersonal skills The ability to work autonomously and manage your own caseload effectively A client-centred and solution-focused approach to case management Strong problem-solving skills and the ability to navigate complex funding and service systems Own mobile phone and laptop An ABN, along with Professional Indemnity and Public Liability insurance A valid driver’s license and access to a reliable vehicle Location: Greater Brisbane Region or Regional (Toowoomba) Start Date: Immediate Ready to Join Us?
